To submit a room reservation request, start by opening the WPC Room Reservation Request Form. You have multiple delivery options.
If you plan to email Lesleigh, edit and save the request, then attach it an an email to Lesleigh_Anello@sbcglobal.net
If you plan to fax, deliver or mail, print and complete the form, then deliver it via:
(1) Fax: 219-838-0702
(2) Drop Off at Church
(3) Regular Mail: 8955 Columbia Avenue, Munster, IN 46321
The date will not be scheduled until you have received confirmation from Lesleigh.
